Tool Kit for Active Living with Chronic Conditions

This evidence-based program includes all tools in the Chronic Disease Self-Management Program, packaged into a one-time mailing. It offers an alternative method for those who cannot or do not wish to attend a CDSMP workshop in person or online. The self-tailored Tool Kit contains:

  • Living a Healthy Life with Chronic Conditions, 5th Edition book
  • Relaxation for Mind and Body CD
  • An exercise CD with accompanying booklet
  • A self-test to help decide how to use the materials
  • Tips sheets on the most important self-management tools.

Learning how to manage your physical and emotional problems can be a challenge, but you can do it! People with chronic conditions need lots of tools and hopefully this Tool Kit contains most of what is needed. As with anything that is new, this may be a bit awkward at first, but success can be achieved by taking it slow and easy. The Tool Kit will suggest and give you some ideas to try.

Who can participate?

This program is open to adults of all ages who are living with one or multiple ongoing health conditions, and cannot or do not wish to attend a workshop in person or online.

What does it cost to participate?

The program is FREE to participants, and participants receive the Living a Healthy Life with Chronic Conditions, 5th Edition book; the Relaxation for Mind and Body CD; an exercise CD with accompanying booklet; a self-test to help decide how to use the materials; and tips sheets on the most important self-management tools.

What happens in the program?

Participants are provided with a one-time mailing of the Tool Kit. This Tool Kit does not provide cures or miracles. Instead, it introduces information and skills that are helping many people with many different physical and mental health conditions lead a healthy life. The Tool Kit will suggest and give you some ideas to try. We are not going to tell you what to do. Rather, you choose the tools that will be best for you, try them, make corrections if necessary, and over time, become an expert self-manager.
