These free, evidence-based programs are for adults living with chronic conditions, chronic pain or diabetes, and include materials packaged into a one-time mailing. The Tool Kit for Active Living Program offers an alternative for those who cannot, or do not wish to, participate in our small group (Tool Kit for Active Living + Calls) or online (Better Choices, Better Health) programs.
Learning how to manage the physical and emotional impact of living with chronic conditions can be challenging.
A Tool Kit includes:
- Living a Healthy Life book
- Relaxation for Mind and Body CD (not in the Diabetes Kit)
- An exercise CD with accompanying booklet
- A self-test
- Tips sheets on important self-management tools
What happens in the program?
Participants are provided with a one-time mailing of a tool kit. The kit introduces information and skills that are helping many people with different physical and mental health conditions lead a healthy life. Choose tools that will be best for you, try them, make corrections if necessary, and over time become an expert self-manager.
